What a GLP-1 receptor agonist is

Glucagon-like peptide-1 is an incretin hormone released from the gut after eating. It binds the GLP-1 receptor, a Gαs-coupled G-protein receptor, stimulating adenylate cyclase and driving intracellular cAMP accumulation and protein kinase A activation. Native GLP-1 is degraded within minutes by dipeptidyl peptidase-4, which is why every synthetic analogue in this class is engineered around two problems: resisting DPP-4 cleavage, and staying in circulation long enough to be useful.

Both problems are solved structurally. Semaglutide uses an Aib substitution at position 8 plus a C18 diacid chain for albumin binding; tirzepatide uses Aib at positions 2 and 13 with a C20 diacid; retatrutide uses alpha-methylated non-coded residues with a C20 fatty diacid conjugate.
